First, please apply two following SAP notes.
SAP Note 1687453 - Output control does not add outputs as expected
SAP Note 1705620 - Output control does not add outputs as expected II
Second , can you please verify that you are using the standard SAP print program when you are experiencing the error. ( SAPFM06P as print program and MEDRUCK as form )
Please read the attached SAP Note 111062 which explains the standard SAP print program.
